This document is an application form (AMED02) for approval as a Medical Examiner of Divers under 'The Diving at Work Regulations (Northern Ireland) 2005'.  

This document (AMED02R) must be completed for renewing approval as a medical examiner of divers.  

This form is to be used by doctors applying for appointment for the first time or by established appointed doctors applying to have their appointment extended to cover additional regulations.

This form is a clinical record (HSENI MS101) under the Ionising Radiation Regulations (NI) 2000.

This form can be used as a surveillance record for a person exposed to lead.

This form is a certification of unfitness/fitness under the Control of Lead at Work Regulations (NI) 2003.  (HSENI MS103)

This form should be completed by appointed doctors wishing to renew their appointment.

This form is a respiratory symptom questionnaire for licensed work under the Control of Asbestos Regulations (Northern Ireland) 2012.  (HSENI MSLW1) 

This form is a health surveillance questionnaire for licensed work under the Control of Asbestos Regulations (Northern Ireland) 2012. 

The questionnaire should be completed six weeks after employment commences and annually thereafter on the anniversary of the commencement of employment - unless the company occupational health adviser determines otherwise.
